Study Plan - Declaration of Major/Minor

Detailed Instructions and Information

The STUDY PLAN—DECLARATION OF MAJOR/MINOR form and instructions, and a Study Plan Worksheet, are available from the UCSC Undergraduate Advising Center: http://advising.ucsc.edu/student/declaration.

UNIVERSITY, GENERAL EDUCATION, AND COLLEGE REQUIREMENTS

It is the responsibility of the student to enter University, General Education, and College Requirements using the Degree Progress Report and the Student Advising Summary, prior to contacting the college for approval.

STUDY PLAN

The academic program(s) approves the STUDY PLAN and admit students to the major(s)/minor(s).  Academic program advising and declaration of major/minor practices vary; contact the academic program(s), and web site(s), for specific instructions.

Review the Academic and Administrative Calendar for the Declaration/Change of Major/Minor deadline for enrollment the following quarter. 

Consult the UCSC General Catalog, and the academic programs, to review program requirements.

Use the Degree Progress Report, and/or the unofficial transcript(s) to complete this section. 

Major, Second Major/Minor(s)
Specify the program(s), i.e. Anthropology B.A., Biology B.S.; History, minor

Lower-division courses (0-99) completed in the major(s) and/or minor(s)
List courses, i.e. ECON 2, PSYC 1

Upper-division courses (100-199) completed in the major(s) and/or minor(s)
List courses, i.e. BIOC 100, LIT 101

Study Plan for the major(s) and/or minor(s)
Propose a course of study to complete the academic program(s), (it is recommended to include all program requirements, GEs, etc., to prepare a comprehensive plan)

Senior Comprehensive Requirement
Specify how the requirement will be met for the major(s), i.e. coursework, examination, GRE, thesis, etc. 

Signatures
Student: sign and date the form
Following the declaration practice of the academic program(s), obtain faculty signature(s), and submit the completed form to the academic program(s).

Students may select the UCSC General Catalog from which graduation requirements are met (known as Requirement Terms).  The Requirement Term default is the quarter of admission to the university, and is subject to certain guidelines; consult The Navigator.

Students may not exceed 19 credits per quarter without prior approval of the college.

PROGRAM APPROVAL

The academic program(s) approve the STUDY PLAN, and the Requirement Term(s), for major(s)and/or minor(s), (the colleges approve individual majors).

If seeking approval for more than one academic program, (double major, major/minor), contact each program, and develop one STUDY PLAN to complete all program requirements.  Approval from each program is required.

EXTENSION OF ENROLLMENT

If the STUDY PLAN requires an Extension of Enrollment, as determined by the academic program(s), college approval is required.  Submit a Petition to Extend Enrollment to the college; the college may request additional documentation.  Program(s) approval is contingent upon Extension of Enrollment approval.  Consult the college for specific information.  

DROPPING A DECLARED PROGRAM

Single major: submit a new STUDY PLAN—DECLARATION OF MAJOR/MINOR.  

Multiple major(s) and/or minor(s): if dropping only one major and/or minor, contact the program(s) directly.
