Students should complete the first section of the Study Plan - Declaration of Major/Minor, filling out their university, general education, and college requirements. 
College Eight Sara Walsh Drop off the petition at the College Office (College Eight Academic Bld, 114) during our open hours (M-F, 9-Noon and 1-4). The petition will usually be ready for pick-up within 3 business days.
College Nine Bob Taylor The Advising Office is located in Social Sciences 2, Room 150. No appointment is necessary and you may see any advisor on a drop-in basis during the hours of 9-12 and 1-4.
College Ten Olivia Chan The Advising Office is located in Social Sciences 2, Room 150. No appointment is necessary and you may see any advisor on a drop-in basis during the hours of 9-12 and 1-4.
Cowell College Cathy Schender Drop off the petition at the College Office. The petition will be ready for pick within 3 business days.  No appointment is necessary, but you may see any advisor during the hours of 9:30-12pm and 1-3:30pm, Monday-Friday on a drop-in basis or by appointment (call 459-2253).
Crown College Crown Advising Bring your form to the Crown College Office (across from the mailroom). You may drop off your form with the college assistant or see an academic advisor. For our walk-in advising schedule or to make an appointment, stop by or call 459-2665. The office is open Monday through Friday, 9 - 12 and 1 - 4.
Kresge College Ursula Oberg Students bring the form to the Kresge College Office to make arrangements to meet with an adviser.  Drop-ins, appointments, and group sessions will be available at various times during the quarter. To schedule an appointment in advance, call the Kresge College Office at 831-459-2071 or instant message kresgeoffice on AIM.
Merrill College Merrill Advising Student brings form to meet with an Advisor. The Advising Office is located in the Merrill College Office. You may see any advisor during daily advising or by calling 459-2144 to make an appointment. Daily advising is open everyday except Thursday from 10:00 to 11:30 and 1:30 to 3:00.
Oakes College Homayun Etemadi Students bring the form to the Oakes College office and meet with an academic preceptor. Students can either make an appointment or come to drop-in advising hours. Sign-up sheets for appointments are available at the Oakes front desk on Mon-Fri from 9-12 and 1-4 (or call 459-2558).

The Advising Office is located in the Porter College Main Office (D-104). No appointment is necessary and you may see any advisor on a drop-in basis during the school year.  Appointments are recommended for multiple and complex programs.

Art Jude Pipes ENTERING FROSH (FALL QUARTER): Declare pre-art at orientation or before the first day of class to be eligible to enroll in lower division studio and foundation classes.

SOPHMORES: Must declare the pre-art major the first quarter of their sophomore year to be able to complete the requirements on time.

JR TRANSFERS: Must pass portfolio review before becoming eligible to declare Art.

The Advising Office is located in the Chemistry Dept. Office, Physical Sciences Building, Room 230. No appointment is necessary and you may see the advisor on a drop-in basis during the hours of 11-12 noon and 1:30 - 3:30 p.m.
Community Studies Joanie Peterson The Community Studies Advising Office is open for drop in advising Monday - Thursday 9-12 and 1-4 in 215 Oakes.

Earth and Planetary Sciences Cathy Smith The Department Advisor's office (Earth and Marine Sciences A251) is open for walk-in advising M-F, 8:30-11:30  a.m. and 1:30-4:30 p.m.  You may also call 459-2504 or email csmith@pmc.ucsc.edu for an appointment.

The Declaration of Major form will also require a faculty advisor's (Patrick Chuang) signature after completing the study plan with the staff advisor (Cathy Smith).  His office is located in E&MS A254 and his office hours will vary each quarter.  You may contact him for an appointment at pchuang@pmc.ucsc.edu.

Declaration sessions are mandatory for those seeking to declare the major/minor. Workshop times, dates and drop-in hours are available on-line (http://econ.ucsc.edu/Ugrad/advise.shtml) and at the department office. Students must have a minimum combined GPA of 2.8 in Econ. 1 & 2 to declare and are advised to bring in verification of their grades, which can be printed off the student portal, to the declaration of major advising session. Students who do not bring in verification of their grades could have their form delayed up to 48 hours.

Film and Digital Media Contact the Film and Digital Media Department. Students are encouraged to visit the Film & Digital Media web site http://film.ucsc.edu/ for more information about the major and minor.

Prior to declaring the Film and Digital Media major, students must complete course 20A, and either 20B or 20C, with a grade of B- or better. Courses 20A, 20B, and 20C must be taken for a letter grade by students intending to major in film and digital media. Students who have met the B- grade minimum for declaration of the major, may choose to take course 20P as the third lower-division requirement.

Once students have met the minimum lower-division course and grade requirement to declare the Film & Digital Media major, the process begins at the college: Students obtain the declaration of major form and  have section i signed by their college advisor.  Once students have met with their college advisor, the next step is to attend one of the weekly declaration of major meetings lead by the Film & Digital Media Department Advisor.  At this meeting, students develop a course-plan for completing the major in the time the student has remaining at UCSC.

History of Art and Visual Culture Elena Lledo Prospective majors must complete two of the lower-division 10-series courses in history of art and visual culture before declaring the major. All students considering this major should consult with the history of art and visual culture undergraduate adviser as soon as possible. Students must complete the worksheet for declaring the major in preparation for a meeting with a faculty adviser to finalize the Proposed Study Plan Declaration of Major/Minor petition form.
